Streamlined portal signup experiment: standard vs. progressive intake

Set up an A/B experiment on the new-member intake form. Control: current 7-step linear form (demographics, insurance, health history, care preferences, notification settings, emergency contact, terms). Treatment: progressive intake showing only 3 fields initially (name, date of birth, email), with remaining fields surfaced after the member books their first appointment. Primary metric: intake completion rate within 24 hours. Secondary metrics: time-to-complete, support contacts during onboarding, 7-day retention. Run for 3 weeks on all new signups, 50/50 split.
