Question 1: 'On a scale of 0–10, how likely are you to use [Platform] for your next visit instead of an in-person appointment?' Question 2 (open text): 'What would make your next telehealth experience better?' Send only to members who completed (not just booked) their first virtual visit. Tag responses with member segment, plan type, and whether the member enabled notifications before their visit.
